- Note where the feeling is physically felt by checking the corresponding body part under "I Feel It In My:" like Head, Heart, Stomach, Neck or Shoulders, Arms or Hands, Legs.
- Rate the intensity of the feeling using the star scale under "The Feeling Is:" by selecting Tiny, Medium, or Big.
- Write down thoughts associated with the feeling in the box labeled "Some Thoughts In My Head Are:".
- If the feeling causes stress, select one or more coping skills from the four categories: Relaxation Skills (e.g., Take 3 deep breaths), Distraction Skills (e.g., Count from 1 to 100), Movement Skills (e.g., Exercise/Run/Jog), or Thinking Skills (e.g., Use encouraging self-talk).
- Optionally, write additional feelings, body sensations, or coping strategies in the blank lines provided.
