- Fill in the Landlord's Name and Mailing Address.
- Fill in the Tenant's Name and Additional Occupants.
- Enter the Property Address.
- Select the Residence Type (Apartment, House, Condo, or Other) and specify if "Other".
- Enter the number of Bedroom(s) and Bathroom(s).
- Choose either "Standard Lease" or "Month-to-Month Lease".
- If "Standard Lease", fill in Start Date and End Date.
- Then choose whether the Agreement terminates or converts to a month-to-month lease after the End Date.
- If "Month-to-Month Lease", fill in Start Date and Termination Period in Days.
- Enter the Monthly Rent amount and the Due Date (day of each month).
- Choose either "No Security Deposit" or "Security Deposit".
- If "Security Deposit", enter the Amount and the number of days after lease termination for returning it to the Tenant.
