- Enter your AADHAAR Card Number (3*) in the provided boxes.
- Provide your full name (4*) under “Shri/Smt/Kum.” as required.
- Enter your Father’s Name (5*) clearly.
- Tick the appropriate box for your Community (6*) — UR, SC, ST, or OBC — and attach the required certificate if SC/ST.
- Enter your Date of Birth (7*) in DD/MM/YYYY format.
- Indicate whether you are a Govt/PSU/Rly Employee (8*), and if yes, provide details including service dates and attestation.
- Mark Yes or No for Ex-Serviceman (9*) and Person with Disability (10*); if yes for disability, specify type (VH, OH, HH) and indicate if scribe is required.
- State whether you are seeking fee exemption (11*) and mention the category number if applicable.
- Attach DD/IPO/Original Post Office Receipt (12*) by pasting the slip on the reverse side of the form; enter number, date, and value.
- Declare if you are seeking Age Relaxation (13*) and mention the category number if applicable.
- List only prescribed Academic and/or Technical Qualifications (14*) with year of passing and subjects/trade/branch.
- Provide your Correspondence Address (15*) in CAPITAL letters only, including Name, P.O., City, District, State, and PIN Code.
- Mention the Nearest Railway Station (16*) if you are an SC/ST candidate.
- Copy the declaration (17*) in running handwriting (not in capitals) affirming truthfulness of information and understanding of consequences of false statements.
- Provide Left Thumb Impression (18*) in the designated box.
- Sign the form (Candidate’s Signature) in non-capital letters and enter the Date and Mobile Number.
- Ensure mandatory fields (Columns 1, 2, 4 to 15 & 17, 18) are completed; leaving them blank will invalidate the application. Write “NA” if a field does not apply.
