- Fill in the date the agreement is made in Section I.
- Enter the Landlord's full name and mailing address in Section I.
- Enter the Tenant(s)' full name(s) and mailing address(es) in Section I.
- Provide the complete Property Address in Section II.a.
- Check the appropriate Residence Type box in Section II.b and fill in “Other” if applicable.
- Enter the number of Bedrooms and Bathrooms in Section II.c.
- Select either Fixed Lease or Month-to-Month Lease in Section III and fill in required dates and notice period.
- In Section IV, check all applicable payment items and fill in corresponding dollar amounts and due dates.
- Specify which utilities are not the Tenant’s responsibility in Section V.
- Add any additional terms or conditions in Section VI.
- Both Landlord and Tenant must sign and print their names in the signature blocks at the bottom.
