- Fill in the date at the top of the form.
- In Section I, provide the Seller’s Name and Mailing Address, followed by the Buyer’s Name and Mailing Address.
- In Section II, enter the purchase price in the dollar amount field.
- Check one box to indicate the type of property being sold: Motor Vehicle, Boat, or Other.
- If Motor Vehicle is selected, fill in Make, Model, Body Type, Year, Color, Odometer Miles, and VIN.
- If Boat is selected, fill in Make, Length, Title #, Hull ID #, and Odometer (hours).
- If Other is selected, describe the item being sold.
- In Section III, both parties must sign and date the document to acknowledge the “AS-IS” terms and absence of warranty.
