Emergency Card Template - Free Printable

- Complete the "Address" section with the street, city, state, and zip code.
- Enter the "Emergency Contact" name and phone number.
- Provide the "Physician" name and their phone number.
- Indicate whether the person wears "Contact Lens" (Yes or No) and specify their "Blood Type".
- In the "Medical Conditions" section, list any chronic illnesses, surgeries, or significant health issues.
- Under "Dangerous Allergies", note any severe allergies to medications, foods, or environmental factors.
- In the "Medications" section, for each drug, record the "Name", "Dose/Color/What for/When to take".
- Add any other relevant medical information in the "Other" field.
- Save the completed card and update it regularly at MediIDs.com as instructed.
